Key Responsibilities:

• Coordinate and manage pharmacy operations to ensure efficient and effective medication dispensing.
• Provide expert pharmaceutical care, including medication therapy management and patient counseling.
• Ensure compliance with regulatory requirements and hospital policies.
• Monitor medication use, identify potential drug interactions, and recommend appropriate therapies.
• Collaborate with healthcare providers to optimize medication therapy and improve patient outcomes.
• Conduct medication reconciliation, order verification, and medication administration record reviews.
• Participate in continuous quality improvement initiatives to enhance pharmacy services.
• Provide clinical leadership and mentorship to pharmacy staff.

🎓 Kualifikasi & Syarat

Kualifikasi & Syarat:

• Bachelor's degree in Pharmacy from an accredited institution.
• Minimum of 5 years of experience in clinical pharmacy practice, with a strong background in medication therapy management.
• Certification as a Clinical Pharmacist Specialist (CPS) or equivalent.
• Proficiency in using pharmacy management software, such as Epic, Cerner, or MedMined.
• Excellent communication and interpersonal skills to work effectively with healthcare providers and patients.
